Online Appointment Scheduling

  • Submitting an appointment request does not guarantee a confirmed appointment. Appointments are confirmed only when you receive written or verbal confirmation from our office.
  • We reserve the right to decline appointment requests at our discretion.
  • You are responsible for providing accurate and complete information when scheduling.
  • Cancellations must be made at least 48 hours in advance. Repeated no-shows or late cancellations may result in a cancellation fee or dismissal from the practice, per our Cancellation Policy.

  • Keep your login credentials confidential and not share them with any third party;
  • Notify us immediately if you suspect unauthorized access to your account;
