Online Appointment Scheduling

- Submitting an appointment request does not guarantee a confirmed appointment. Appointments are confirmed only when you receive written or verbal confirmation from our office.
- We reserve the right to decline appointment requests at our discretion.
- You are responsible for providing accurate and complete information when scheduling.
- Cancellations must be made at least 48 hours in advance. Repeated no-shows or late cancellations may result in a cancellation fee or dismissal from the practice, per our Cancellation Policy.

- Notify us immediately if you suspect unauthorized access to your account;
